The innovative model of care includes pharmacists

The Congress of the European Society of Cardiology (ESC), currently held in Amsterdam, brings together specialists from around the world and has a prominent Spanish representative (eighth country in terms of the number of studies presented). Within this framework, sessions on innovative treatment models in the field of cardiovascular disease have presented positive results from various collaborative schemes in which pharmacists are a key element in improving the health outcomes of patients with this type of pathology. .

In his opening press conference, John V. McMurray, from the University of Glasgow (England), mentioned heart failure as one of the main topics of this scientific event.

His colleague Ricky Turgeon, of the University of British Columbia (Canada), presented this Sunday in an innovative treatment scheme session the evaluation of PHARM-HF, a working formula for titration of heart failure therapy led by pharmaceutical professionals.

A similar approach is taken by Meysam Pirbaglou (Toronto, Canada), who has spoken of alliances between (other) healthcare professionals and physicians to strengthen heart failure therapy according to clinical guidelines. This is a strategy that aims to “expand our therapeutic opportunities,” he explains.

Emily Turner’s team from the UK have presented a paper on how to increase the frequency and correction of direct acting anticoagulant (DAA) regimens in a pharmacist-coordinated model.

Turgeon described the results of this latest study as “impressive” in terms of reducing errors and improving anticoagulant therapy for patients with atrial fibrillation. The two key indicators were the patient’s CHADS-VASc score and the reduction in medication errors since joining the program.
